Insurance
To better serve our growing patient populations, we have contracts with most managed care insurance plans. Please help us serve you best by studying the specific requirements of your own insurance plan as each plan is different.
We will gladly file claims with your primary carrier. To facilitate this, bring your current insurance card to each and every visit. Our staff will reconfirm this information at each appointment. However, payment of services is ultimately the responsibility of the patient.
Please be prepared to pay your co-payment before each visit. We accept Cash, checks as well as Visa and Master Card.

Made 24 hours in advance, If you are more than fifteen minutes late, you may need to reschedule. We understand that emergencies happen and we are willing to work out any personal matters you might have.
We ask you for your understanding and patience when we are running behind schedule. Emergencies occasionally disrupt the schedule. When this happens, we will try to get back on schedule as soon as possible.
During office hours you may leave a voice mail message for a specific staff member or speak with a receptionist if your question or problem is complex, we would prefer that you make an appointment for an office consultation...

Prescription Refills
Please allow 48 hours to refill your renewed medication, please check prior to your appointment to see if you are running out and refills will be handled at the time of your office visit.
Most prescriptions, for short-term problems need no refills; however, if a refill is authorized, please call your pharmacist and ask the pharmacist to fax us a refill request to (972-701-0201). New Prescriptions of antibiotics or potent medications will not be handled by telephone. This could be dangerous to you without proper medical evaluation.
Confidentiality
All of your medical records, office notes, and correspondence are absolutely confidential. Information will be released to your spouse or family members only with your prior written authorizations. This is the law. Family members are always welcome to accompany you on your visit here if you wish. Reports concerning your health will not be released without your authorization. Under Texas State code, your record may be accessed by an attorney requesting information for any legal suit you are or may become involved in. |
